Which is the most important area to include your keywords?

1. Place your target keyword in the page title. Keep it as close to the beginning as possible.

2. Place the focus keyword in the URL.

3. Place the combination of focus keyword and LSI keywords in the meta description.

4. Place the focus keyword in H1 tag and use focus keyword/LSI keywords in H2 - H6 heading tags.

5. Use the focus keyword in the first 100 words.

6. Include keywords in image alt text.

7. Include a combination of focus and LSI keywords throughout the content. Don’t over optimize it.

8. Use keywords for anchor text as well but...

This basically sums all the important areas where keywords need to be added, also ideally keyword density should be around 1-2%, and relevancy of keyword usage should be given most importance.
